Life threatening illness can cause health to decline. Being in familiar surroundings offers profound comfort. Crossroads Hospice customizes each person’s care to suit their chosen living environment whether it’s a nursing facility, assisted living or their own home.
Hospice – or “comfort” – care is used to describe an approach to healthcare that is specifically designed to support the physical, emotional and spiritual needs of those who have life-limiting illnesses or conditions.
Hospice can be offered in a patient’s home or a care facility. It is designed to provide emotional support to patients and families, and to educate caregivers so that they are able to provide physical care to their loved one. Hospice care provides compassionate care and support for those in the last stage of life so that they may live as fully and comfortably as possible.
Hospice is a physician-directed, nurse-coordinated, interdisciplinary approach to patient care. Hospice team members can include a registered nurse, a medical director, a home health aide, a social worker, a spiritual care coordinator, a volunteer and your attending physician, if he or she remains involved in your care.
Crossroads Hospice providers are available 24/7.
Crossroads Hospice is committed to providing exceptional end of life care to all those in need. Our goal is to guide patients and caregivers in achieving physical, emotional and spiritual dignity, quality and compassion to those we care for by honoring their choices, wishes and beliefs.
